Below are several forms which can be downloaded to your computer and used to price our products, place orders, contact us, request information, etc. Also included is an Art Sheet which can be used for image placement purposes. The order forms are in two different formats. The standard 8.5 x 14 legal size order form has all the information you need to fill out to place an order. Please notice all the information areas which are needed to correctly enter an order with us. Graphics placement is very important to insure that the customer gets exactly what they are expecting. Also Trim Colors and Garment Styles Numbers need careful attention.

The other Order Form is an interactive online order form which can be used for placing orders for ONE GARMENT STYLE ONLY. This order form is a dynamic PDF form which does everything for you except input the information needed to complete an order.
